Planning for Discharge or Relocating
Planning for Discharge
Discharge planning prepares you and your child for a smooth transition after reaching services goals or if/when ABA services are no longer appropriate. Your child’s ABA provider develops a discharge plan when they create your child’s TP.
Common reasons for interruption or discontinuation of ABA services include:
- Relocation
- Choosing not to continue ABA services
- ABA services are determined to no longer be needed by your child’s ABA provider
TRICARE East will provide approval for parent training for six months during your child’s discharge transition.
Relocating
TRICARE East can assist you when relocating:
- Notify your ASN about your move as soon as possible
- If moving within the East Region, your ASN will help find a new provider
- If moving outside the East Region, your ASN will coordinate with the new region and transfer all ACD-related documents
