Paying Enrollment Fees

You can pay your fees annually, quarterly or monthly. Before deciding to pay annually, consider that enrollment fees are non-refundable, in most cases. One exception is if you become eligible for Medicare under age 65 and decide to purchase Medicare Part B.

In this scenario:

  • You can stay enrolled in the US Family Health Plan and your enrollment fee is waived.
  • Any enrollment fee payments you made can be refunded.
  • You should pay monthly or quarterly to avoid unnecessary overpayments.
Payment Type When you Submit Your Enrollment Form: 

Annual Payment

  • Single: $282.69
  • Family: $565.20
  • Your fee is prorated from your enrollment date to December 31st.
  • After that, your annual payment is due on January 1st each year.

Quarterly Payment

  • Single: $70.65
  • Family: $141.30
  • Your quarterly fee is prorated to cover the period until the next calendar year quarter.
  • After that, your quarterly payment is due at the first of each quarter.

Monthly Payment

  • Single: $23.55
  • Family: $47.10
  • Pay your first three months with a personal check, cashier's check, traveler's check, money order or credit card.
  • After that, set up a allotment or electronic funds transfer (EFT) to pay your monthly enrollment fees.
