What is a Post-Hospital Care Program?

This program helps TRICARE East beneficiaries in certain Prime Service Areas (PSA). It works to reduce unnecessary hospital readmissions and emergency room visits.

Will Someone Contact Me If I’m Identified?

Yes, a care manager will call you to explain the program. If you agree, the care manager will set up an in-home visit. During this visit, the care manager will check for safety risks, review your hospital discharge instructions, look for signs of infection, and help you schedule follow-up appointments.
