What is a post-hospital care program?
This program helps TRICARE East beneficiaries in certain . It works to reduce unnecessary hospital readmissions and visits.
What is in-home case management?
After a hospital stay, in-home case managers will check in with you at home to support your recovery. They help to make sure you understand your discharge instructions, coordinate any follow-up care or equipment, and watch for early signs of complications. This prevents small issues from turning into another hospital stay.
What kind of support can I expect from the program?
- Gives you a single point of contact who already knows your case
- Helps schedule follow-up appointments and makes sure referrals and equipment arrive on time
- Checks on how you are recovering at home
- Connects you to community resources beyond what TRICARE covers
Will joining the program affect my TRICARE benefit?
No. In-home case management is a voluntary service offered at no extra cost. If you choose not to participate, it will not reduce, delay, or change your TRICARE benefit or coverage. You can decline the service or stop participating at any time. You can also opt in later if your needs change.
Am I required to participate?
No. Participation is always your choice. A case manager may reach out after a hospital stay to offer support, but you are free to decline or ask to be contacted at a different time.
What happens if I decline now but want help later?
You can request a case manager at any time. Declining once does not remove you from future eligibility.
