Case Management

We offer Case Management services for eligible beneficiaries with special health needs. These services support people of all ages, from children to adults. Our Case Managers help guide you through the process and make it easier to access services in the TRICARE East Region.

Goals of Case Management

• Teach you about health conditions and needs
• Help you find resources for healthcare and make discharge easier
• Ensure you get timely authorizations
• Work with your doctors on prescriptions
• Make sure you understand your prescribed medications
• Simplify transitions to other levels of care, facilities, or regions
