Find/Change My Primary Care Manager
What is a primary care manager?
Your primary care manager is responsible for your routine, nonemergency, and urgent health care. If your PCM can’t provide your care, they’ll refer you to a specialist. Your PCM can be a military provider or a civilian network provider (if a military provider isn’t available). If you are enrolled in any of the TRICARE Prime options below, you will choose or be assigned a PCM.

- TRICARE Prime Overseas
- TRICARE Prime Remote Overseas
- US Family Health Plan (you’ll have a primary care provider. A PCP is the same as a PCM).
- TRICARE Prime Demo
Note: Sometimes your PCM may change without your control. For example, when your military doctor changes duty station or retires.

Call the provider first to see if they’re accepting new patients.
How do I change my PCM?
To change your PCM:
- Log in to milConnect.
- Call your regional contractor.
- Mail a TRICARE Prime Enrollment, Disenrollment, and PCM Change Form with the new PCM’s name and address to your regional contractor. You only need to complete part of the form related to the PCM change. Use the form associated with your plan below.
You may change your PCM at any time. Your request may take up to six days to confirm. The military hospital or clinic can approve or deny your request. You can check the status of your change request by logging in to milConnect.
