Once you agree to participate, you’ll receive a link to chat via email and/or SMS text message — whichever you prefer. Simply click on the link to start a secure chat from any computer, tablet, or smartphone that can open a web link.

The Diabetes Type 2 Program is for adults who want to improve how they manage theirtype2diabetes.Personalizedchatshelpwithself-monitoring,includingtracking your blood sugar results and reminding you to take the medications prescribed by your doctor.
The Hypertension Program is for adults who want to manage their high blood pressureathome.Personalized chats help with self-monitoring, includingtracking yourbloodpressureandremindingyoutotakethemedicationsprescribedbyyour doctor.
ThePostpartumCareProgramisforadultwomenwhowantsupportduringthe12 weeksafterhavingababy.Personalizedchatshelptomonitorbloodpressureand offer breastfeeding support, as well as other recovery and care issues that new moms experience.
TheLifestyleManagementProgramisforadultswhowanttomanagetheirhealth and wellness. Personalized chats help with lifestyle changes including exercise, nutrition, routine screenings, and weight management.
