How to Submit a Claim Appeal

You have the right to request a reconsideration of a claim denial.

What do you disagree with? Write a request to explain why you want the denial reconsidered. Be sure to sign and date the request.

For care on or after Jan. 1, 2025, you should send the request by mail or fax no later than 90 days from the date of the Explanation of Benefits notice.
