If you live in the U.S., District of Columbia, Puerto Rico or U.S. Territories, you have 1 year from the date of service or inpatient discharge to file your medical claim. If you live overseas, you have 3 years from the date of service or inpatient discharge to file your medical claim. You must also submit proof of payment with your claim.
