Active Duty Dental Program Claims

United Concordia’s network dentists will file claims for you. But if you’re authorized to visit a non-network dentist, you may have to submit your own claims.

TRICARE Dental Program Claims

• Participating dentists will usually file claims for you.
• If you need to submit a dental claim, mail or fax a claim form and supporting documents to United Concordia.
• Forms and address are based on where you got care: the CONUS service area or the OCONUS service area.
