Request Archived Health Records

If you need a copy of your health records, your local military hospital or clinic can provide one. You can only get copies of records related to a hospitalization, fetal tracings, or ambulance services (collectively referred to as “inpatient records”) from the facility where you got this care. Be prepared to give your full name, sponsorThe term sponsor specifically refers to the active duty, retired or deceased service member whose military status entitles their dependents (spouse, children, etc.) to receive TRICARE health benefits.’s Social Security number, DoD ID number, and place and dates of your last treatment information.

If you're a/an Follow these steps

Active duty service member
  • Download the DD Form 2870 and follow the instructions.
  • Submit the completed DD Form 2870 to the relevant military hospitals or clinics.

Veterans or next-of-kin of a veteran seeking health records from their time in the military
  • Active duty family member
  • Retiree or retiree family member
  • Former active duty family member

And you're eligible to get care at military hospitals or clinics
  • Download DD Form 2870 and follow the instructions.
  • Submit the completed DD Form 2870 to the relevant military hospitals or clinics.
 
  • Active duty family member
  • Retiree or retiree family member
  • Former active duty family member

And you're no longer eligible to get care at military hospitals or clinics
  • Download NA Form 13042 and follow the instructions.
  • Mail the completed NA Form 13042 to the National Archives and Records Administration.
  • If NARA doesn't have your health records, contact the relevant military hospitals or clinics where you received care. It's possible your health records haven't been transferred to NARA yet.
