Use the TRICARE Prime Enrollment, Disenrollment and Primary Care Manager Change Form (DD Form 2876) to enroll in one of the TRICARE Prime options: 

• TRICARE Prime
• TRICARE Prime Remote
• TRICARE Prime Overseas
• TRICARE Prime Remote Overseas

To enroll by mail, download, complete, and mail your region’s form to your regional contractor:

• East Enrollment Form
• West Enrollment Form
• Overseas Enrollment Form

Enroll Online

• Go to milConnect.
• Click the “Sign In” button at the top of the page. You must have a Common Access Card, DFAS (MyPay) account, or myAuth account to log in.
• Once logged in, select the “Medical” tab to enroll.
