How Can I Change my PCM?

If you want to change your primary care manager to a network provider, call the provider first to check if they accept new patients. You can change your PCM anytime, as long as the new PCM accepts new patients, and your request follows local military hospital or clinic rules.

Request a Change Online

If you want to select a civilian PCM or change to another PCM within the military hospital or clinic, submit your request through Beneficiary Web Enrollment. If you choose the default date, which is six days in the future, the change will take effect retroactively to the date you submitted the request. If you select a different date, the change will take effect on that date.

You can also mail a completed TRICARE Prime enrollment and PCM change form. You may contact a customer service representative for help. If you choose to mail your request, complete the sponsor section, the PCM change section, the signature section, and the Access to Care section, if needed. The change will take effect when we receive the PCM change form.

You might have trouble viewing the PCM change form in your web browser due to security settings. If this happens, right-click in the new browser window and select "Save as" to save the form to your computer. You can then open it using your default PDF viewer. You can also access the form directly from the official DOD forms site.

Receive Confirmation

After we process your PCM change, you’ll get an email or postcard with instructions on how to access milConnect for the new PCM’s name and phone number. All change requests follow the military hospital or clinic commander’s guidelines. PCM changes take effect when we receive the request. Please wait for confirmation of the change before seeking services.
