Case Management
If you're being treated for chronic, high-risk, high-cost, catastrophic or terminal illnesses, you can get case management services at no extra cost.
Case managers are usually nurses or social workers who can help patients and families figure out complex health care and support systems.
To locate a case manager, contact:
- Your regional/program contractor
- Your military hospital or clinic
- A Beneficiary Counseling and Assistance Coordinator
Are you moving?
Talk to your child’s case manager and regional contractor before you move. They can help you find a new doctor and other resources so the transition is smooth with no interruptions in coverage.
