What is the US Family Health Plan (USFHP)?

 

The US Family Health Plan (USFHP) is a TRICARE Prime option. You must be TRICARE-eligible to enroll and live in a USFHP service area. If you’re an active duty service member, you can’t enroll. To learn more, visit the US Family Health Plan page.

 

Costs:

  • If you are an active duty family member, you don’t pay an enrollment fee or out-of-pocket costs for covered services as long as you get care from USFHP providers.
  • All others pay an annual enrollment fee and copayments.
  • Use the TRICARE Compare Cost Tool to see your costs (from the Plan drop-down menu, choose TRICARE Prime.)

 

Providers:

  • You’ll have a primary care manager (PCM) who provides most of your care.
  • You can’t get care from TRICARE network providers or at military hospitals or clinics.

 

Referrals and Authorizations:

Your PCM helps you get appointments with specialists in the area and coordinates your care.

 

Covered Services:

Pharmacy Benefits:

You may use USFHP participating pharmacies. You can’t use military pharmacies. 

 

Dental Benefits:

Vision Benefits:

Available but depends on who you are (for example, active duty family member, retiree), and the type of vision care you need. To learn more, visit the Vision page
