How do I change my primary care manager?

 

To change your primary care manager (PCM):

• Call your regional or overseas contractor;
• Use the Beneficiary Web Enrollment via milConnect; or
• Send a completed TRICARE Prime Enrollment, Disenrollment, and PCM Change Form to your regional contractor with your new PCM’s information
