How do I report the death of a beneficiary?
You can either:
- Call the DMDC/DEERS Support Office at 1-800-538-9552; or
- Make an appointment at the nearest ID card office.
Note: You will need a copy of the death certificate.
To file a claim on behalf of the deceased, visit the Filing Claims page for claim forms and instructions.
- For TRICARE For Life beneficiaries, you need to submit the TRICARE For Life Executor/Executrix of Estate Notification with the claim.
- For beneficiaries in all other plans, submit an Authorization for Disclosure of Medical or Dental Information (DD Form 2870) with the claim.
If you’re reporting the death of a retired sponsorThe term sponsor specifically refers to the active duty, retired or deceased service member whose military status entitles their dependents (spouse, children, etc.) to receive TRICARE health benefits., there are extra steps you need to take. See below.
