How do I add or delete my other health insurance information?

Download the Other Health Insurance (OHI) form for your region or plan. Fill it out and send it to the address on the form.

If you have:

• Other pharmacy coverage, call the pharmacy program contractor to let them know, 
• TRICARE Dental Plan (TDP) coverage and other dental insurance, call the TDP contractor to let them know; or
• Coverage through a Federal Employees Dental and Vision Insurance Program (FEDVIP) dental plan and other dental insurance, call your FEDVIP dental plan to let them know. 

Be sure to keep your other insurance status up to date to avoid denials or recoupments.
