How do I get a copy of my medical records?
It depends on the type of record and where you got care.

You'll need to request a physical copy from your military hospital or clinic's records management office. Visit the Request Archived Health Records page to learn how to get copies of your archived records. |
If you got care from a civilian provider (including overseas) you must request a copy from each individual health care provider. You may have to pay a fee for copying or mailing your medical records. Note: If you are an active duty service member, you aren’t responsible for fees charged for copying records of authorized care. If charged, you can file a claim with your contractor for reimbursement. |
