TRICARE East Payment Options (East Region)

TRICARE East offers multiple options to pay your premiums, depending on your plan. You may pay with a debit or credit card, via allotments from your retirement pay, or electronic fund transfers from your bank account.

EFTs or allotment payments are the safest, no-hassle ways to pay your monthly premiums. TRICARE Prime and TRICARE Select retirees must pay monthly enrollment fees via allotment, when feasible. Recurring credit card payments and one-time payments are also available.

Automatic Credit/Debit Card Charge

Use this form to establish automatic payments with your debit or credit card for TRICARE Prime enrollment fees or monthly premium payments for TRICARE Reserve Select, TRICARE Retired Reserve, or TRICARE Young Adult.

The address and fax number for submission are on the form.

Enrollment Fee Allotment Authorization

TRICARE Prime beneficiaries should use this form to start, change, or stop automatic enrollment fee deductions from their military retirement pay.

This form is completed online.

Monthly Automatic Electronic Funds Transfer From Checking or Savings Account

Use this form to establish automatic payments for TRICARE Prime enrollment fees, or monthly premium payments for TRS, TRR, or TYA using EFT.

Note: If you miss a payment and don’t pay it within the necessary timeframe, the contractor will disenroll you from your plan. Call Humana Military if this happens.
