  • Active duty
  • National Guard or Reserve
  • Retired

Active duty  
  • Ranks E-4 and below: $64 per individual, but no more than $128 per family
  • Ranks E-5 and above: $193 per individual, but no more than $386 per family
National Guard or Reserve 
  • Ranks E-4 and below: $64 per individual, but no more than $128 per family
  • Rank E-5 and above: $193 per individual, but no more than $386 per family 
Retired 
  • $193 per individual
  • No more than $386 per family 

Cost-Shares

After you meet your deductible, you’ll pay a cost-share based on the type of care you get and type of provider you see (network or non-network).

Your CHCBP cost-share depends on your beneficiary category before your sponsor separated:

  • Active duty family members (includes separated active duty service members)
  • National Guard and Reserve members and their family members
  • Retired service members, their family members, and other beneficiaries
