Why did I get a third party liability form?

 

When your doctor files a claim, he or she has to provide information on your visit. Doctors do this by using one or more diagnosis codes which describe your injury or illness. Some diagnosis codes may indicate an injury or illness which a third party may have caused. When the TRICARE contractor gets claims with these types of diagnosis codes, the contractor will send you a Statement of Personal Injury – Possible Third Party Liability form (DD Form 2527) to fill out. To learn more, visit the Third Party Liability page.

Note: You must send the form back within 35 days or the contractor may deny your claim.
