Our patients are at the forefront of a healthcare revolution in primary care. Our vision is to proactively improve your health through the patient-centered care provided by our medical home staff. The medical home model launched here in 2009 and is now being implemented throughout the Military Health System (MHS).
We support a model where the key prescription is to encourage and impact healthy behaviors. We group our staff of 250 healthcare professionals into medical home teams, where patients receive a collaborative, comprehensive and holistic set of healthcare solutions from the providers on that team. The teams include not only staff internists, but also dieticians, tobacco cessation specialists, psychologists and mind-body therapists. And our suite of technology tools helps improve access to care so same-day appointments can be made for patients who need them.
We serve active duty and retired military personnel, and including those needing geriatric services. We also provide routine and preventive care, consultative services for patients with complex medical conditions, inpatient care, and acute care (sick call) for active duty military personnel.
Our Disease Management Service is available to patients enrolled in our clinic who have a chronic medical condition. An experienced nurse educator provides counseling and support sessions to patients with conditions including diabetes, hypertension, dyslipidemia, polycystic ovarian disease (PCOS), heart failure, Parkinson’s disease, pre-diabetes, weight management issues, and more. The need for case management services is determined through a collaboration between you, your family or caregiver, your health care team, and your provider. This collaboration will include a comprehensive assessment of your health, psychosocial needs, and use of health care services and resources.
