Admissions:

To be admitted to the hospital for an overnight stay, you need to provide Admissions with:

  • Patient’s full name, date of birth, address, gender, religion, marital status
  • Patient’s military ID card
  • Sponsor’s name, rank, rate, social security number
  • Name and telephone number of person to be notified in case of emergency
  • Copy of your Advance Directive (such as a living will or durable power of attorney for health care), if you have one
  • If you have insurance besides TRICARE, then your health insurance card is needed.
  • If the admission is for a motor vehicle – related accident, then auto insurance information is also needed.
    • Collecting insurance information helps us recover medical costs. All funds recovered from insurance companies are used to improve patient services.

What To Bring and Not To Bring for an Overnight Stay at the Hospital:

  • We suggest you bring: personal care items (such as pajamas, robe, slippers, toothbrush, toothpaste, shaving cream, razor). If you’re unable to bring sleepwear, we’ll supply it.
  • You can bring mobile phones, laptop computers, and tablets to access the wi-fi connection. (You’re liable for all personal electronics.)
  • Please don’t bring over $10 cash or valuables. Valuables will be inventoried and deposited in a safe, and you’ll withdraw them when you’re discharged. (We’re not responsible for valuables not deposited with the valuables custodian.)
  • Don’t bring any medicines (any prescriptions, or over-the-counter items like vitamins). Your doctor will prescribe all medicines you’ll use while you’re an inpatient. Don’t take anything else, as that can cause negative interactions.

Emergency & Urgent Care:

The hospital is our only location with an Emergency RoomThe hospital department that provides emergency services to patients who need immediate medical attention. (E.R.). Branch health clinics don’t have E.R.s.

Our hospital’s E.R. is open 24/7. Using the standard “triage” model, patients are seen based on the seriousness of their condition, not the order in which they arrived.

An emergency is a medical, maternity, or psychiatric condition that would lead someone with average knowledge to believe that a serious medical condition exists; that the absence of immediate medical attention would result in a threat to life, limb or sight; severe painful symptoms that require immediate attention to relieve suffering; or when a person is at immediate risk to self or others. Examples of emergencies include: severe bleeding, chest pain, severe eye injury, broken bone, inability to breathe, spinal cord injury, or no pulse.

    • Cholesterol and lipids tests require you to fast for 10 - 12 hours beforehand.

Dad or significant other is welcome to stay the night, and visiting hours are 24/7.  We offer couplet care (mom and baby together), breastfeeding counseling from our lactation nurses, “Siesta for the Fiesta” (daily quiet time to support you in feeding your baby), newborn hearing screening, a 24/7 educational “newborn channel” on your in-room TV, and a state-of-the-art infant security system.  For safety, you need an installed car seat, before leaving the hospital.

  • Birth Certificates & DEERS Update

Before leaving the hospital, you’ll complete a birth certificate application with Admissions. Admissions will electronically file it with Duval County Health Department’s Bureau of Vital Statistics. Multi-Service Unit:

This is one of our inpatient units.  To ensure patient privacy, please check with the nursing station before entering a patient room.  Visiting children must be healthy and supervised by an adult at all times.  In some cases, we recommend that the very young or elderly not visit, so patients aren’t exposed to possible infection.  The number of visitors might be restricted, based on patient condition or room size.  Patient privacy is important to us, and we respect the patient’s right to request limits.

Rapid Response Team:
