Before you retire, request paper copies of your medical record and your family members’ medical records.  

Medical Records from a Military Hospital or Clinic

You may request paper copies of your medical records from the administration or records office at your military hospital or clinic.

Medical Records from Civilian Providers

You must request a copy of your medical records from each individual health care provider you have visited. You may be responsible for any fees charged for copying or mailing medical records. Active duty service members are not responsible for any fees charged for copying or mailing medical records. 

If you would like another party to have access to your medical records, please complete an Authorization to Release Information form.
