Care Companion is an interactive chat tool that helps you better manage your unique needs between medical appointments. Think of it like a personalized digital companion. Health care experts designed the automated responses to ensure you get the right care at the right time.

Want to know more? Here’s how it works:

  • Care Companion is currently available for patients managing type 2 diabetes, hypertension, postpartum care for new mothers, and lifestyle management.
  • Upon enrollment, you’ll receive a link to a chat via email. Simply click on the link to start the secure chat from any computer, tablet or smartphone that can open a web link.
  • Ask questions and receive answers tailored to your unique health needs. 

Care Companion is only available at the five venture sites at this time. 

The five venture sites include:   

  • Eglin Air Force Base Hospital, Florida
  • Martin Army Community Hospital, Fort Benning, Georgia
  • Naval Hospital Jacksonville, Florida
  • Naval Medical Center Portsmouth, Virginia
  • Wright Patterson Air Force Base Hospital, Ohio

No. Care Companion is an automated chat tool designed by health care experts to provide responses based on information entered by you. 

No. Care Companion does not use artificial intelligence.

No. There is no cost to participate in a Care Companion program.

No. Using Care Companion is optional and does not replace the opportunity for you to speak directly with your care team. Care Companion can help you better manage your health between medical appointments.

Patients are enrolled in specific Care Companion programs based on certain health criteria. If you are enrolled, you will receive an email invitation with a link to begin your Care Companion chat. Simply click on the link to start a secure, personalized chat.

Once you are enrolled in Care Companion and receive your email invitation, you can immediately begin to use the program by following the link in the email.

No. Care Companion uses your web browser and does not require an app to participate.

No. Care Companion will work with any web browser. Chats are accessible on any computer, tablet, or smartphone that can open a web link.

Yes. You can call the appointment line or your local military hospital or clinic to schedule a phone appointment with your health care team.

Care Companion gives you the flexibility to manage your health condition or get answers to questions at a time and place that is convenient and comfortable for you. Care Companion can help you manage your health concerns and provide personalized information in between medical appointments. 

Care Companion is currently available for patients managing type 2 diabetes or hypertension, as well as postpartum care for new mothers and lifestyle management. Other programs to help manage chronic conditions and health concerns will become available in the coming months.

The Diabetes Type 2 Program is for adults who want to improve how they manage their type 2 diabetes. Personalized chats help with self-monitoring, including tracking your blood sugar results and reminding you to take the medications prescribed by your doctor.

The Hypertension Program is for adults who want to manage their high blood pressure at home. Personalized chats help with self-monitoring, including tracking your blood pressure and reminding you to take the medications prescribed by your doctor.

The Postpartum Care Program is for adult women who want support during the 12 weeks after having a baby. Personalized chats help to monitor blood pressure and offer breastfeeding support, as well as other recovery and care issues that new moms experience.

The Lifestyle Management Program is for adults who want to manage their health and wellness. Personalized chats help with lifestyle changes including exercise, nutrition, routine screenings, and weight management.

No. Currently programs are only available in English.

No. Currently Care Companion chats are not captured in your electronic health record.

If you are enrolled in a Care Companion program you can continue using the program regardless of whether you transfer to a different health care team.

Care Companion does not replace the relationship you have with your health care team. Care Companion can help you better manage your health between medical appointments. You should continue to meet with your health care team to manage your health concerns, when appropriate.
